The Administrative Simplification provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA, Title II) requires the Department of Health and Human Services (HHS) to establish national standards for electronic health care transactions and national identifiers for providers, health plans, and employers.

 

This means that many health care and similar facilities now require your health care surrogate or agent to also have a HIPAA form signed by you indicating you give your permission to have this person access your medical records.
