What is an Organ Donor Form?

You can express your desire to be an organ donor in a variety of documents and Rainbow Law provides a specific form designed for this purpose.

 

In addition to this form, you may want to carry on your person a donor card, an indication on your driver's license, etc.

 

Methods for notifying a desire to donate your organs vary from state to state. Some states require completion of a form such as the one we provide in our packages.

 

Organ donations may also be indicated in a Will (although this method may make it difficult as the Will does not take effect until you die and your donation must take place within hours of your death) and in your advance directives.

 

Regardless of your state's requirements, it's important that your family and loved ones know of your desire to donate your organs. Planning for such a possibility in advance and making your decision known increases the likelihood of your wishes being followed upon your death.

It's also a good idea to provide a copy of your organ donor documentation to your health care provider and hospital to be kept with your medical records.
